outreach & transition support

Central to well-being is the availability of good, supportive relationships.
Research evidence highlights that relationships are pivotal to helping children and young people, both in care and after leaving care, to thrive and flourish. Therefore, we place a strong emphasis on helping young people build strong relationships and networks of support while they are with us and beyond.
Listening to young people’s views the message is clear: relationships are pivotal to their quality of life and emotional support is as vital as practical help, but they also tell us how the care system and those who support them don’t always prioritize these
Bright Spots program- CoramVoice- Our life, Beyond Care
We offer Outreach and Transition Support programs for children when they move on from our homes, whether they are returning to family, moving to a foster placement, transitioning to independent living, or accessing other specialist services.
Every transition is carefully planned with the child’s needs and preferences in mind. Our team works closely with the child and their support network, including their social worker, to create a seamless and supportive transition process.
  • We believe that children in care should have the same opportunities to maintain relationships with primary carers when they move on as other children leaving family homes.
  • We recognise that the initial transition period can be especially challenging emotionally, often feeling exciting, stressful, and scary all at once.
  • Therefore, we aim to ensure that young people have continuity of support from trusted carers and adults in their wider network during this critical time.

Young Person

CASE STUDY

Individual outreach package
We provided long-term outreach support to a young person who, unfortunately, required psychiatric in-patient admission due to a rare, treatment-resistant mental illness. This young person had previously lived with us for over a year, receiving mental health support in the community.

Our ongoing support ensured continuity of relationships with key carers, helping them through this challenging period until they were able to transition to more specialized community health care. We also collaborated with medical staff to help them better understand the young person's needs, progress, and potential. This support included weekly visits to the young person, as well as participation in multi-agency meetings.
